Use this link to access the schedule of upcoming NREMT Basic exams.

NREMT Basic Exam Request Form  (Word)
This form is used by EMS coordinators to request a practical exam date. The form includes choice of Examiner, exam location, number of candidates and date of exam.

NREMT Basic Exam Testing Materials
The items listed below are the items that an Exam Coordinator must print.  The number of copies necessary are at least one for each student and additionals for any student that may retest.  All grids are two-sided, please make sure you copy both sides.  Each grid or grid set must be on a different color, preferred colors are listed following the document name.  Please do not print the grids more than seven (7) days prior to your exam.

    Practical Report Summary is now available in two formats.  The first is completed by hand.  The second is a form with only the student information is completed by hand.  We are also asking that the students provide a phone number in case we have questions.

    Roster, Signature page, Evaluator list, Pass/Fail form and Dispatch

EMS NREMT Roster-Eligibility Form Windows XP or Windows 2007 (Excel)
This template is used by EMS coordinators to list the eligible candidates and generates other forms necessary for EMS testing.  Which template you use is based on what operating system you have on your computer.

It is imperative that you open, print and read the INSTRUCTIONS (PDF) (additional instructions for 2007 users) prior to opening the template.  The instructions explain what to do if you get a "pop-up warning" when you open the template, as well as explains how to deal with other warnings you may encounter while using the template.  The instructions will also explain the use of the special icons needed to finalize the process.

      WTCS.
